Process
Aid to individuals:
1. Individuals applying can fill the form on this site,
Or
Write to us on WhatsApp or email, and we will send a link to the form, which you can use on your phone.
(To submit the form, you will need to upload your Aadhar Card and Estimate/Application Letter/Prescription – the form will guide you on what to upload)
2. We will send you a form and a link on WhatsApp. You can print the form, fill it and sign it. You can then submit the filled form and other necessary documents using the link given to you.
We will put your case up to the Trustees. Some applicants may need to discuss their Prescription with a Staff member or a Trustee on the phone.
On the meeting date, we will phone you if we have any questions.
4. After the meeting, we will inform you by WhatsApp of our decision, which may be:
- Immediate Grant
- Grant Letter
OR
- Denial.
The Next Step:
5. If you are given an Immediate Grant, the amount will be sent directly to the hospital/school.
If you are given a Grant Letter, then:
- the Grant Letter will show the maximum amount sanctioned
- the Grant Letter must be used within 3 months
- after 3 months, you may request a Grant Letter.
6. When you are ready to be admitted to the hospital, you must inform us by sending us Proof of Hospital Admission Date and Documents that show how you will meet the cost.
- We will then decide the amount to be sent directly to the hospital or school.
- This amount could be less than the original grant.
